assess the patient properly if what is the needs that will need nursing management like for impaired skin integrity it may com with pain, your nursing consideration is to alleviate pain.

always remember not to deviate from your nursing intervention to medical yan palagi yung mistakes ng mga student especially if you got MS books...
parang na copy paste lang nila ang sa books...

always consider turning the patient on the unaffected side q4h and keep the area clean, this also contributes in alleviating pain.

Correct. Ang NCP nag fo focus on the HERE and NOW nung pasyente. Kung alagaan mo sya ngayon at nabigyan na pala ng sangkaterbang pain meds at wala na yung pain nya, hindi na priority ang pain. Kaya nga first part nung NPC is assessment kasi dun ka gagawa talaga ng diagnosis mo.

Malay mo yung pasyente mo namomroblema sa pambayad pala sa ospital at di na pala makakain, walang kinalaman directly sa decubitus ulcer pero its a good priority for ncp.

Well, sa physical therapist point of view try mo silipin ang Prevention. Ayaw mo  na lumawak pa yung decubitus ulcer ng patient. So ayan yung turning schedule mo (max po yung every 2 hours, pero ideally every 30-45 minutes), infection control, etc. Possible problem din pero depende sa specific diagnosis yung activity niya. Baka immoblized siya. Dapat be aware of immobilization syndrome. ROM exercises ang pantapat diyan.

kung awake at may stroke lang
1. impaired physical mobility or pain depende kung may verbalization yung patient mo na pain or grimace
2. impaired skin integrity
2. altered nutrition

kung psychological effect naman kung walang problem ang patient sa ABC ito ang unang priority
1.kung body image disturbance kung preoccupied sya sa decubitus at ayaw pakita sayo.
2.pwede din low self esteem sabi ni paine kung wala syang pambayad (kung ito ang problem pwede first priority ito)

pero sa mga cases na ganito multiple ang problem hindi lang decubitus either combination yan ng pneumonia at stroke patients, minsan mga coma patients
