NSO Birth Certificate

mga lawyer dito tulong po

I have this problem my NSO birth certificate has clerical errors wrong spelling ung name and middle name ko.. sabi dun sa LCR namin na it will take 3-5 months and 1 month if always na followup sa NSO manila. what could be the best way?

And can u give me a sample of Affidavit of proof for my BIrth Certificate that has different speelings?

parang ang alam ko dyan bro, every letter na papalita will cost 1,000 or something d ko lng sure ha, well yan lng ang alam ko about wrong spelling sa birth certificate!

pwede din yata mag apply ka lang ng certificate of late registration tapos lagay mo na correct name mo doon with of course its correct spelling..pero di ako sur..try mo na lang..

Coordinate ka na lang nga sa Local Civil Registrar nyo. Sanay na sila dyan. May list of requirements and supporting documents sila na ibibigay sayo (SSS, BIR, Brgy Clearance, NBI, Proof of Billing, Company ID, Diploma, Certificate of Employment), dapat yata sa mga supporting documents mo ay tama ang spelling ng name mo para yun ang pruweba na may mali nga yung COB mo. After nun, kailangan mo pa mag pa publish sa news paper na mag che-change ka ng name due to clerical errors. Sa pag publish, mas mura yung mga news paper na contact ng local registrar nyo, valid un, kaso nga lang ndi sikat na dyaryo. After pa lang nun at saka nila pwedeng i process yung request mo na correction. 3 months na nga yung pinakamabilis nun. Pero nag i-issue naman sila affidavit na may pending request ka to correct your name.

P.S. Ndi rin ako lawyer. :D

Post ko na din to..in my part naman misis ko wrong spelling name na naregister ng lolo nya sa munisipyo nila.Now she tried to ammend it with the name she uses since the begginning but the municipal office is asking for more than P7,000+ na bayad for processing. Ok lang ba ung mga ka spies.thanks

Hindi puwede ito dahil magiging falsification of public document. Dahil naka register.

Kung simple na clerical error puwede na mag coordinate sa local registrar. Ex. Mali lang ang spelling. But if ibang name talaga ang naka lagay ibang usapan. O kaya instead of male, female ang naka lagay.

Kung malaki you may need to go to court. Consult a lawyer, the lawyer will file the necessary pleading sa proper court. Tapos bayad ka nang publication sa newspaper. medyo magastos. Kung walang mag oppose sa pleadings, tuloy tuloy na iyan.

affidavit lang yan, like sa case ng sister ko all her life Diana ang gamit nya, sa school records, sa PRC sa everything, pero ng kukuha sya ng passport ang lumabas sa NSO ay DAYANA, nag pagawa lang sya ng affidavit sa office of the mayor ng town namin saying that her name is DIANA and not DAYANA tapos attached nya sa NSO birth certificate, yun okay na, and up to now yun ang gamit nya in case she needs to present a birth certificate, like sa NCLEX sa marriage contract... hope it helps...
